Referral Process Information

SuperSibs! Referral Intake Flow
  1. Referral Comes In:
    Intake Call or Receipt of Referral Form via mail, fax or e-mail
  2. Diagnosis is Confirmed:
    • If referral is from known hospital personnel, this step is already completed
    • If referral is from other source, we confirm that the child(ren) meet our criteria for: age, diagnosis, and residence within the US or Canada.
  3. If Confirmed:
    Welcome Kit is sent to family
    (Parent welcome Kit with personalized, individual Kids’ Packages inside)
  4. IF NOT Confirmed:
    Call is made to the person who made the referral to further explain program, refer to other organization, etc.
  5. Enter into Database:
    Referral information is entered into SuperSibs! secure, confidential Database and coded so that we can track their status (Active, Opt Out, Alumni, Lost – Need New Address, Bereaved, etc.) and track sibling mailings throughout their involvement with the program.
  6. Ongoing Mailings:
    Kids will receive age-appropriate SuperSibs! Comfort Packages in the mail approximately 10 times a year.
  7. Annual Follow-Up to SuperSibs!
    In December, we send a Certificate and an Evaluation Survey to each sibling in our program, and a survey to each parent.
  8. Maintenance:
    • SuperSibs! must be notified if a patient has died so appropriate SuperSibs! bereavement comfort mailings can be provided, as these siblings continue to receive support in our program.
    • SuperSibs! must be notified if the family has moved, so mailings will be received by the siblings/parents.
    • Families can “opt out” of SuperSibs! when they feel the siblings no longer require our support services.
    • When a child turns 19 they are moved to “Alumni” status and will receive a letter and final evaluation form.
    • When a family is referred into the program with only children under age 4, they receive a letter letting them know that when their child turns 4 he/she will be entered into the program. At this first contact, the young child receives a small acknowledgement package and will receive their full welcome kit upon officially entering the program.
    • If the family has multiple children – and one/more are under 4, the welcome letter indicates that our ages are for 4 – 18…. Those children under 4 will be entered into the database and will be monitored until they turn 4 and become “Active” in status. All siblings between 4-18 receive their Welcome Packages immediately upon referral.
